The  Field Based   Community Health Worker is responsible for assessment, planning and implementing care strategies that are individualized by patients and directed toward the most appropriate, least restrictive level of care. They also Identify and initiate referrals for social service programs; including financial, psychosocial, community and state supportive services, and manage the care plan throughout the continuum of care as a single point of contact for the member. As a  Field Based   Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. The coordinator also addresses social determinants of health such as transportation, housing, and food access.

Schedule:  Schedule is Monday through Friday an 8-hour shift to be determined by the hiring manager between the hours of 8 am to 5 pm Eastern Standard Time.

Location:  Macon, Swain, Jackson, Haywood Counties, and surrounding areas North Carolina.

\n

Local travel up to 50% and mileage is reimbursed at current government rate.

\n

Primary Responsibilities:

  • Linking members to applicable community resources
  • \n
  • Complete field visits for high risk, hard to reach members
  • \n
  • Telephonic outreach to targeted members
  • \n
  • Keep the members out of the hospital by supporting regular visits to their primary physician
  • \n
  • Keep members actively engaged with their primary physician
  • \n
  • Support the member to ensure pick - up of their Rx
  • \n
  • Proactively engage the members to manage their care
  • \n
  • Provide member education
  • \n
  • Support transitions of care
  • \n
  • Create a positive and engaging experience for the member
  • \n
  • Help to keep members compliant with their care plans
  • \n
  • Collaborate with members’ care team (community, providers, internal staff)
  • \n
  • Partner with and support care team by completing delegated tasks
  • \n
  • Knowledge and continued learning of community cultures and values
  • \n
  • Conduct individual-level screenings to capture members’ current health care needs (e.g. Transitions of Care, Health Risk assessments)
  • \n
  • Engage members to participate in the screening process and collaboratively develop plan of care based on their individual needs, preferences, and objectives
  • \n

Required Qualifications:

    \n
  • High School Diploma/GED (or higher) OR 2+ years of community outreach experience
  • \n
  • 1+ years of community outreach and/or field work experience in a health care setting
  • \n
  • Intermediate level of computer proficiency including MS Word, Excel and Outlook
  • \n
  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) and access to high-speed internet
  • \n
  • Must have resided within the local community for 2+ years with familiarity of culture and values of the community
  • \n
  • Valid state of North Carolina driver’s license and state-required insurance with access to reliable transportation that will enable you to travel locally approximately 50% of the time and up to 150-mile round trip to client and/or patient sites within a designated area
  • \n
  • Reside in North Carolina within a commutable distance to Swain, Macon, Jackson, and Haywood Counties
  • \n

Preferred Qualifications:

  • Bachelor's Degree in a health-related field or social work or Health Care Administration
  • \n
  • Community Health Worker (CHW) State course completion
  • \n
  • CHW certification
  • \n
  • Licensed Practical Nurse (LPN), Certified Nursing Assistant/Home Health Aide, Certified Medical Assistant
  • \n
  • Experience with arranging community resources
  • \n
  • Experience with electronic charting
  • \n
  • Background in managing populations with complex medical and behavioral needs
  • \n
  • Knowledge of Medicaid population
  • \n
  • Bilingual – Spanish
  • \n

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
